All coaches and players are required to have Identification Badges.  Player badges must be presented to the Referee prior to the start of the match. A coach badge must be worn for the duration of the match.

Game Card and ID Options- (Game cards need to be printed on Card stock & ID should be laminated)
Game Card & Player IDs#1 -  This is a fillable option. Enter all of your information on the roster page, the information will flow onto game cards and player IDs.
Game Card #2  - Basic game card
Extra Coach ID -  Fillable and laminated

Game cards are required for all matches.  Each coach should fill out a match card for their team prior to each match.  The card must be filled out completely and turned into the referee prior to the start of the match along with the Identification Badges.  Players' full name should be entered in jersey number order.  The referee will retain the cards and turn in to the host region after the match.  The host region is required to maintain the match cards until after the post-season tournaments have been completed.
